This resource demonstrates the journey a young person leaving the care system can take. It is aimed at both practitioners and young people moving towards more independent living and young adulthood.

It takes account of the changes introduced by the Children and Young people (Scotland) Act 2014 and is intended to help practitioners to apply good, transparent practice, and informs others about these key processes.
