This page is about consent to take part in Together's State of Children's Rights Report 2024.

Together requires a completed consent form for each baby, child and young person taking part.

We have three different categories of consent form - please expand the tabs below to access the relevant forms.

For babies, toddlers and any child unable to give their own consent
Consent form to be completed by parents/carers.

For children and young people aged 15 and under
Consent form to be completed by both the child/young person and their parent/carer.
